Flexible Spending Accounts: Employees can set aside up to $5,000 on a pre-tax basis for eligible medical expenses and $5,000 pre-tax basis for eligible dependent care expenses (full-time employees).

Life and AD&D Insurance: The Standard Life Insurance Co., company-paid insurance coverage for twice an employees annual earnings (full-time employees).

Supplemental Life Insurance: The Standard Life Insurance Co., voluntary insurance coverage (full-time employees).

  • Employee Life Benefit - an employee can elect up to three-times his/her annual salary (up to $500,000)
  • Spouse Life Benefit - an employee can elect up to $50,000 in $5,000 increments. Must be purchased in conjunction with Employee's additional life insurance.
  • Dependent Life Benefit - $10,000 or $20,000.

Long Term Disability: The Standard Life Insurance Co., company-paid benefit which provides an employee with 60% of his/her salary to a monthly maximum of $15,000 (full-time employees).

Short Term Disability: The Standard Life Insurance Co., company-paid benefit which provides exempt employees 100% of salary and non-exempt employees 60% of salary (full-time employees). *Part-time employees receive state disability benefits.

Employee Assistance Program: Horizon Health EAP Services, company-paid benefit which provides employee assistance and WorkLife services for employees and family members (full and part-time employees).

Adoption Assistance Program: Adoption eligible expenses reimbursed up to a $10,000 lifetime maximum. Employees are eligible for reimbursement after completion of 12 continuous months of service (full-time employees).

Tuition Reimbursement Program: Tuition and registration fee expenses reimbursed up to the annual IRS Cap. Amount of reimbursement is based upon final grade. Employees are eligible for reimbursement after completing 6 months of service. Employees must work toward a work-related degree or certification (full and part-time employees).

Gym Reimbursement Program: Company will subsidize a fitness program for a maximum of $60.00 per quarter ($240.00 annually). Employees are eligible for gym reimbursement after the completion of 6 months of service (full and part-time employees).

Weight-Loss Reimbursement Program: Company will subsidize a weight-loss program for a maximum of $60.00 per quarter ($240.00 annually). Employees are eligible for Weight Loss Reimbursement after completion of 6 months of service (full and part-time employees).

Vacation: Vacation time is determined by length of service and must be approved in advance. Year one vacation time is calculated as of hire date, resulting in a prorated amount of time for the first year of employment (after the first 90 days). From year two forward, vacation time is calculated on an annual calendar basis (part-time employees will be prorated based upon hours worked.)

Paid Sick Leave: Upon completion of first 90 days of employment, 40 hours of paid sick time per year is accrued for non-exempt employees. Exempt employees are eligible for unlimited sick time suring a calendar year (part-time employees will be prorated based upon hours worked).

Personal Leave: All employees are eligible for 2 paid personal days per year (part-time employees will be prorated based upon hours worked).
